I've generally seen that framed as "modern internet available in the US is behind other countries" so still sorta focusing on the middleground of service, or the higher end. Not the lowest end, which is equal parts an infrastructure and monopoly problem.

No telecom would refuse money from the other side of the street just because.

I dunno sometimes their reasoning for not extending who's wired up amounts to "we wouldn't instantly make the money back it would take a little while so meh"

They seem pretty content to not expand or upgrade at all and just find new ways to BS existing customers into paying more for less.
