Short answer: Atmosphere

Long answer: How the game adds to the sense of urgency as you go. The moon getting closer is an obvious one. Then you've got characters that go from oblivious to the apocalypse, to worried, scared, and some even reflective on what they've done or haven't done. And then there's things like Clock town theme getting faster as time goes and adding ominous sounds as you get closer. And that's before the final six hour theme, which is a masterpiece in of itself.

Like you need the clock to tell you exactly how much time you've got left, but the game just perfectly let's you roughly know whats going on without the clock.
