Its very good for a 5 year old in that he can draw a discernible face with eyes and mouth and in the correct location.

Its also worrying for a few reasons.

The lack of colour, it could be he only had one pen or he chose to only use one colour which is unusual for kids.

The face that looks like its shouting placed over the smiling face, almost like it's dominating it.

The dark scribbling coming out of the shouter's mouth like some pit that could swallow you whole or its just a very loud noise, so loud its like a physical thing that you can see and touch.

The intensity of the dark mouth, to the point where he tears through the papers. Its obsessive.

Maybe he has someone in his life constantly screaming at him.
