The countries with the most reason to fear the devastation of a nuclear attack are those who don't have nuclear weapons. Nuclear-armed rival superpowers are unlikely to push the national self-destruct button by attacking each other.

If Russia used nuclear weapons against Ukraine, would America really unleash nuclear retaliation -- in consequence of which, as we all know, tens of millions of Americans would inevitably die -- simply for the sake of avenging Ukraine? (Almost) certainly not.

But you have to keep bluffing. The sooner someone realises he can win a war with nukes and very probably get away with it, the sooner he'll try it.
