Some comes out the top (which is what you put your mouth on) and some comes out of the tube in the side (which is where you light the tobacco or whatever it is you're smoking out of it) It passes through the lower pipe into the water which then gets drawn up toward the top where your mouth is, then you tug the lower pipe out of its resting place to let air through so you can suck up all the smoke.
