There's a real world Silent Hill which you ironically see very little of in the games. It's a normal town with a bit of a tourism industry and a bit of a drug problem.
There's a Fog World Silent Hill that the majority of the games takes place in. Roads are blocked off or broken, the entire place is abandoned, buildings are often in ruins, and monsters roam.
There's an Otherworld Silent Hill. This area typically reflects the psyche of its victims more than the Fog World does alone: Alessa's is rusted and bloody, James Sunderland's is dank and mildewed, Murphy Pendleton's is prison-like and industrial. It's typically in complete darkness and there are often areas where the roads have been replaced with grates that hang over an empty void.
This. SH has three layers. Most people never see beyond the sleepy tourist town.
