As far as the rest, of course, economies of larger scale have an advantage. Now, if you let, say, California and Wyoming be in the same market, it's very doubtful CA would see much change at all. On the other hand, WY could see a significant change. I don't buy the state lines thing as some panacea for the US because my sense is that overall, the state lines are not having a very big effect on health insurance now.
