I guess the difference is an accidental death via fall from a ladder is an accident, and being beaten, bludgeoned and burned to death over a book, isn't.

Under hierarchy of control, most, if not all accidents are preventable. What controls can be put in place for random murder?

The number one control on the hierarchy is elimination, the second is substitution.
