So are we assuming he's a good enough shot to get an automatic kill?
Yes, the shooter will be making two kills.

Because it's not the dying that would scare me, it would be living through the shot in either intense pain or surviving with long-term brain damage or physical disability.

Snap your fingers and everything goes black? That's a fine death. It's lingering on that's scary.

Yes, it'd be insta-kill.

Also, "$20 million, but you must play a lethal game" makes it sound like you get the money regardless, and the game only determines if you'll be alive to spend it. So does that mean you get to pass it along to your next-of-kin/designated heirs?

Because the 33% chance of dying becomes a lot less of a threat if it means your family will still be taken care of. Doubly so if you're already old, sick, or disabled to the point where your own quality of life isn't necessarily that stellar as-is. Getting the opportunity to opt-out while simultaneously making sure your loved ones will be supported might be a win-win for a lot of people (hence why life insurance policies tend to have really strong restrictions for suicide).

Regardless of outcome, you get the money and it can be passed on to beneficiaries.

Also:The game must be played by January 2026.
