If all the people that are targeted are nefarious, infamous individuals instead of just tons of regular criminals, all that people will see is lots of people dying of heart attacks. No way to connect back to the Death Note with just that. It would still be suspicious for a bunch of high profile people to suddenly die of heart attack. And there would likely be an investigation into the matter.
