They hired a guy as a manager right out of college, with no experience working in the field. I actually can't remember him doing poorly, but he stepped down into a lower supervisor position with in like a month, and then stepped down from that, too. He was very clearly not prepared for that type of role (he was a good guy though).
Years ago when I applied for a low-level supervisor spot, they gave it to some one else. She was terrible. Hardly did any thing, hardly showed up, ended up getting a higher up manager fired because they fucked. She came in one day with a black eye because his wife punched her. So yeah, good call! (I'm actually not bitter about it, she was not a good choice, but I don't think I was at the time either. I have been promoted multiple times since then)
I'm sure there are more. It happens pretty often in retail, I guess.
