Black Mushroom is one of the items needed for the staff that opens up the secret level
that's what I figured. iirc it was on the ground somewhere in a corner of a dungeon/outside area in act 1. The other thing (I looked at it but forgot again. I think it was Chatting Gem or something) came off of a random unique monster in an ice cave in act 3. It's also a white item with no stats and a message ("makes you want to talk" or something like that)
