It's the first stage, Mario games always look kinda like that in the first area. There's some more diversity in later levels.
NSMB and NSMBWii both used the exact same world designs. Not to say we know that NSMB2 will do that too.
Still I think the problem people have is that honeslty, NSMBWii didn't add enough variety to the gameplay from NSMB1 except for multiplayer, which IMO was what made the game worth playing. I get the complaints because of the fear that it could end up being really derivative.
