Here in Us, the patient (or patients family member/decision maker) usually makes the treatment decisions and we have to obide by it, and we sometimes dont have the balls to say no due to fear of litegation/poor patient satisfaction. There are many times where I have family mebers who want 3 peg tubes, 2 trach tubes for a 95 year old lady that is demented and has no quality of life. We think its wrong, but we have to do it. In this case, its hard to tell but however, I give credit where it is due to the primary team to be able to pick this up just by physicial exam and lab tests, as in this case since the decision maker is primarily providing the history, and the patient is not reliable to provide one accurately. --- |

What they do is the agency would assign nurses where they are needed the most (in this case, places with many COVID cases, hotspots etc) and offer a surcharge for those hospitals, so the RN gets paid more.
