Is it a matter that they don't have gender/sex, or that various demons fill that same role? If within universe different beings serve as pinhead, I guess that's an understandable adjustment?

I haven't seen all the movies but they were all humans once so they do have a sex. the women are clearly women and no attempts were made to make them look genderless. It's various demons filling the same role because they all died in the second movie. Woman or transgender playing pinhead isn't controversial unless you're sexist and hate women.

How many heroes have super strength, robot suit, or telekinesis? Probably a thousand but nobody accuses them of being lazy attempts of making a new character.
