Im sorry, but I think as a doctor you should very well know that if you want to help and educate anyone, you need to have a decent attitude. If I acted like you do in my pharmacy, no one would take me seriously. Its extremely unprofessional.

So I see two broad possibilities.

First, hes real. He seriously talks to himself like that and finds that okay when its definitely not. This is bad.

Second, hes doing a gimmick. But its an extremely involved and dedicated gimmick making so many topics just to have multiple multi-month conversations with himself. This is just as bad.
