It probably involves a lot of arguing about making sure everything looks good. I notice Pac-Man is always yellow with colored armbands in his alt costumes instead of the obvious changing his skin color.

They also have to do this with first party characters from what Sakurai has said. I'm guessing it varies depending on the creators, but apparently some of them have considerable sway over like how Mario jumps and stuff like that.
