Can only talk about my place, but:

There isn't always a fee, especially not at shopping malls.

It's usually 50cent for the cleaning lady at the place, and I think people with intestinal disorders need it especially clean, so... Yeah, just pay it.

Honestly, people are complaining about the wrong thing with the US tax system. It isnt that the government knows how much you owe, its that the government is wrong about how much you owe and you have to tell the government you owe less through countless deductions and tax credits. In other words, the government only knows how much you made, and the tax you owe regarding that; nothing else (e.g., you have a dependent = tax credit, you installed solar panels = tax credit, you paid interest in student loans = tax deduction).

So my solution is to just get rid of every single deduction and tax credit, and then lower the tax brackets accordingly so that the tax rate youre taxed at is what your effective tax rate is. But this will NEVER happen because of special interest groups (like clean energy companies will want tax credits for taxpayers when they buy high efficiency products).

Im not an expert on how UK tax system works, but there generally is only one personal allowance of ~GBP12k, and thats it (no other deductions or credits, which the average US tax payers have a ton of, thanks to special interest groups). So based on that, the effective tax rate seems pretty simple and its just the tax brackets. For UK:

In China, you have to pay and bring your own toilet paper tho (per a Chinese coworker I asked a few weeks ago when that Chinese viral watch an ad for toilet paper video; i.e., the ad for TP is a backup since everyone is expected to carry their own TP in public). Idk about other Asian countries.

In the UK there are disabled toilets that can only be opened with a key. People with those kind of issues are usually given a key for them.

If I didnt have spare cash on hand that would have been inconvenient to say the least.

I was in Ben Thanh market last year and had to pay 5k dong to drain mine. It's only a few cents but yeah, without cash or an ATM machine close by in those situations you're screwed.

It's not real monetization. You can't expect profits with 50 cent per person, it's mostly to keep vandalism down.

In shopping malls it's free, and they are usually clean.
In restaurant / cafes it's free (for consumers), and they are a roll of the dice, because it obviously depends on them.

Public ones can be everything, Depends on the place. The "bigger" the place (popular shopping street, big train stations) the more likely it has the 50 cent barrier and is spotless. If it's some public bathroom at some small park, it's probably free but probably not as nice.

McDonalds sometimes has asks 50 cent here too, and they are still terrible.

Also, be aware that Europe is not a monolith. Every country and even the cities differ a lot there. Ask 5 europeans from 5 countries about such stuff, and you usually get many different answers.

Cant tell you about China, as I havent been there. However, I can say that paying to use a toilet has not come up during any of my time in Vietnam, Thailand, Cambodia, Laos, South Korea, Japan or Malaysia.
