Basically Mandy teaches Billy about the practice of trepanning, or drilling a literal hole into your skull. She convinces Billy let her do it to him, causing him to bleed out

Y'all remember that anime-inspired Billy and Mandy fan comic

And how part of the backstory was that Mandy the story about Billy taking Grim's job for a day and wanting to give it a try

And when she did, it turns out she did 9/11

And when she grew up, Grim married her and to have a kid, Mandy banged some dude and they did a super late-term abortion and adopted the resulting soul that Grim reaped
