When you get modded there should be an "do you accept this moderation, yes or no?" option and if you say no, the moderation and associated punishment should be deleted from the record.

it should have the mod give his reasoning why he moderated you
then allow for 1 question that mod should faithfully answer honestly (or get demodded)
after this you can then choose to dispute it
as it stands disputes are usually information gathering and by the time you have any idea wtf is actually going on

I think having the ability to have a dialogue with the mod without "disputing" would be welcome.
Granted I don't get modded much anymore, but back when I did I remember it seemed silly that I only had like 2-3 potential responses before the ability to dispute was removed

I mean, I can count the number of times I've been modded on one hand, but... I remember always burning my first response just trying to figure out what the heck I was moderated for.

Also the ridiculous "you can't dispute this because you disputed something else" rule needs to be dropped immediately. It's completely stupid, not to mention unfair to members. The outcome of one moderation shouldn't take away your ability to appeal a completely separate argument.
