He left cash for a really good tip though along with an apology note on the receipt

I've been to restaurants yes.

No but I've been drunk enough to go back the next day to make sure I had paid my tab. I had paid it and this only happened once at one local haunt.

Yes, but I credit it to their mess-up. A couple years back I received the wrong dish at a dine-in restaurant and was billed for the dish I was supposed to get. Later that night I looked up the prices and found that this wrong dish was about $5 more than what I'd ordered.

That's not really a dine-and-dash, though. And most places, if they screw up your order, will either partially or fully comp your meal. If they accidentally give you a more expensive item, almost none of them will actually charge you the higher price for it (because that's a really good way to get customers extremely upset at you).
