I'm playing it for the first time now, and after 6ish hours I think only about the 1st hour's worth of the original game's plot has happened.
It's been a lot of side quests to flesh out the slums, and some segments that weren't in the original to give more screentime to Avalanche's members + introduce new characters. But yeah, even if you just skip the side quests, the MSQ is still about 50/50 original stuff and new stuff so far.
