Oh yeah, just follow Miller's advice after you beat her rather than Ocelot's. Could never bring myself to do it but you see it on speedrun videos because you don't have to wait for the chopper afterwards.

Mission 45 is a pretty huge change in pace and I can see why people wouldn't like it. If you've got an upgraded CGM that's definitely the mission to bring it with you/call it in. Battle dress too.

Make sure you keep the aggro on you and away from Quiet; it's a secondary objective to prevent her from taking any damage but she can and will die causing a mission failure if she takes too many tank shells. The entrance to the palace closest to where you start that mission is a good place to hole up. Pop in and out and take out the enemy armour as a priority; soldiers generally won't cause you much of a problem but keep an eye out for any that are getting close. If you get time in between waves and remember where they come from, you can run out and lay down mines so you can ignore one or two vehicles but that takes some practice.

Most importantly, don't forget your HQ functions in the field. You can call in resupplies yourself rather than waiting for Quiet to drop them, and the scouting function will help you pinpoint the enemy. I think you can also issue orders to Quiet so make sure you move her around if you find she's becoming too big of a target.

Lastly, that mission is a lot of fun on replays when you have the parasite suit and aren't worrying about your rank!
