As seen on the 4th disc in the Japanese International release, the Orthopedic thing is a mistranslation.
Based on unused text in the game, originally Tifa had been working herself up at the Honey Bee Inn to get the attention of Don Corneo, and the slightly stretched panties was an item that Cloud could use to crossdress.
