Worst thing I regularly see in the men's room is that guys, for whatever reason, don't use the urinal and won't lift the seat when they pee, leaving yellow spots all over it.
Worst I've seen, unisex, is explosive diarrhea where the person did not sit down first. All over the back wall. And some on the ceiling.
