It's hard to explain but essentially you have challenges you must do while in UVHM that will unlock wildcard versions of previous missions, which each mission unlocking a new UVHM. There's only five levels vs bl3 ten mayhem levels. The scaling for UVHM in BL4 is also way less extreme than mayhem levels. UVHM 1 begins at 150% enemy health and you take 10% more damage.
