Something like this. I actually work in the restaurant industry. When I order from where I'm employed I almost always tip because these are my coworkers and it's hard to stare them in the face the next day if I walk out the bar after giving them goose egg for taking my order.
Other places though? I only tip if I ask them go an extra mile and they execute flawlessly, or make up for any errors by rectifying them and having a great attitude. Tips aren't owed, they're
earned

Also always draw a diagonal line spanning the entire length of the gratuity section if you don't plan on tipping, people. It's exceedingly difficult to corrupt that into a number. If you want to be even safer on the line below where it says the total write in exactly what the bill total is. No credit card fraud possibilities ever.
