There was an Elseworld story set in an alternate 1900, The Batman of Arkham , where Bruce Wayne is a psychiatrist at Arkham Asylum by day while doing his Batman thing at night. He's actually shown to make a lot of progress by treating the patients there like human beings instead of like animals (especially with Killer Croc, of all characters) before he suffers a bad case of Joker Toxin-induced mania, gets locked up in the asylum himself as a result, and a non-costumed Jonathan Crane takes over as head doctor there and reinstates the old, pointlessly cruel, Bedlam-esque ways.I think there is also a modern setting story where Bruce is locked up in Arkham as a patient. I think in that story, all the batman stuff is in his head and he's been insane since the night his parents died. I can't remember the name of the story though and I never have read it fully just seen some pages of it
