In Guild Wars my most prized possession was a Bone Dragon Staff with req. 9 Channeling. The best drop I ever got in that game was a req. 9 Silverwing Recurve Bow. It dropped from the dungeon chest during a PUG for daily bounty quest.

In Diablo 2 IIRC the rarest drop I ever got was a Crown of Ages. It dropped from a random mob in Cathedral of Flames.

Man, I actually can't think of any super rare drops I've gotten. I guess I found a really good relic greatsword in MH4U but it wasn't the best possible one and the whole relic system was garbage.

edit: While not the rarest item, I did have a particularly rare weapon EARLY in FFXI - it only drops from Cerberus... I think it's called Acheron or something like that. I was able to get it in a relatively brief window when there was a way to solo Cerberus with pet damage even if horribly geared (taking like 40 minutes)

The apkallu minion in FFXIV, or, at least back when I had earned it, it used to be super rare. I don't know what percentage of players have it now, but at the end of ARR, it was something like 0.6% of active minion collectors who had it.

A shiny Skarmory in Pokemon Emerald. I wasn't able to catch it though. Any other shinies I was able to attain was through chaining or trading.

I've played through Earthbound a dozen times and think I've only found it once and that was when I was purposefully hunting for it. Pretty sure I hit max level before I got it.

Probably my F/SN UBW room in PSO2? It wasn't super expensive at the time (well.. it was still pretty expensive), but if I were to sell it now, it'd be worth more than everything else I own combined because it hasn't been rerun in years. Too bad it's bound to you upon using it, but that means its price is absolutely nuts now.

I don't remember what it was, but I sold it for something like 25 million meat in Kingdom of Loathing.

If achievements count, I have the "It's Something" and "Some Like It Salty" achievements from American Truck Simulator, each of which only 0.4% of players have. (You get "It's Something" for finding a sign that says nothing., and you get "Some Like It Salty" for making deliveries from every company in Salt Lake City. Both of these should be pretty easy to get, I don't know why more people don't have them.)

Totally forgot that I've had that twice. Once in Pokemon Silver in like 2001 and then again in Diamond in like 2008.

I also bred a Chansey with Heal Bell. But that wasn't really rare, so much as you had to go to ridiculous lengths to obtain one.

Overall I've never really gotten anything particularly rare. Never even got a shiny that wasn't story related.

It's a rare drop off an enemy that is a pain in the ass to farm.

The PS2 version of Final Fantasy XII, using the "alternate" method to get the Zodiac Spear. I wasn't even reading a guide at the time, I just entered entered the Henne Mines (and just so happened to have a Diamond Armlet equipped) and got the spear on the first try.

3rd age longsword in OSRS. I think it's like 1/100000 - 1/300000 chance.

I also received the farming pet from a herb yield, that's also around 1/100000 I believe.
