Based on the definition of hacked, it sounds like they hacked the account. Phishing might be how that got the info in some cases. But it does only say how they got info for some of them. To say some weren't hacked would be right as some probably were. It just says how they got in information in some cases... Some being the key word...

Actually, the best passwords are catch phrases instead of ones with the obtuse password requirements. The guy who designed the original requirements for passwords wishes he hadn't. He didn't read the actual recommendations he was given and regrets it now.

I'd rather have a password that's super long and needing to be looked at but also less feasible to be brute forced, than one that I could remember mentally but is more susceptible to brute force.

But then again, none of this would matter if people simply didn't reuse the exact same combos on all their accounts. XD

Most of my passwords are 2-3 random unconnected words, followed by 2-3 random numbers, followed by 1-2 random numbers.

Though I've also played around with mnemonics - so, for instance, you take a memorable sentence (like, say, "four random words is the ultimate password"), and reduce it down to only the first letters (so "frwisup"). The positive is that it can be easy to remember while hard to back-engineer, the downside is that if it's too iconic to your personality, people who know you well enough to guess it.
