There's 3 main ways Cut open underneath the breast/ along a line from the nipple down then insert them. Insertion through the areola area, not too different from what you described. And then side insertions with tools to position everything, like through the armpit or belly button. The latter isn't viable for larger ones, the middle can have weird areola results but I'll probably want mine stretched in the process of the surgery so they match better so that's probably how I'll go. How little you realize you need upper body strength for almost everything becomes apparently after breast surgery. Over or under the muscle?
