Adventure II for 2600.
In fairness, it wasn't ever officially released for the system but it found its way onto one of those knockoffs with the pre-installed games. So I pick up a key or something and as I'm walking with it, the bat comes out of nowhere to take it away but the game glitches out and I'm now carrying the key and the bat. A few other things get stuck to me and then the key turns into the dragon for some reason. But it's just kinda chillin there being carried around until I come across another dragon whoch I guess was actually the real dragon. I quickly leave the screen when suddenly the dragon I'm carrying because the key turned into it comes to life and kills me. --- ... Copied to Clipboard!

this is really hard to think of for me... I've played a lot of not very good games but I can't remember playing any that were like, offensively bad so nothing stands out that much

Outside of things that tend to be innately bad (like movie and cartoon tie ins)... Probably Shadow of the Colossus.
The titan fights were pretty good at first, but it got repetitive and it's status as an "art" game didnt negate the boredom of its emptiness for me.
