There's optional difficulties and extra items and stuff you can get for replays but personally that didn't click for me, once you beat the game there's no reason to replay it. I haven't played Madhouse myself, but it sounds like a really good way of doing a hard mode. Rather than just making enemies do more damage and your guns do less, they drastically change things around. Just one small example:
