Using Lego Digital Designer. So far I've got Mabe Village more or less done, which is where I originally planned to stop, but it's addictive so I might just carry on and see how much of the island I can do before I go mad.
Anyway, the village still needs a few tweaks, but here's the map so far- note the hideously not-to-scale Link, as I went with one 2x2 brick per tile, making doors (actually windows) about half the size they'd need to be for a minifig to go through

It'd be nice if we could a more... uh, overhead? view of your's, Jawa. You can't really see some things like the entrance to the Thrifty game store or the entrance to Marin and Tarin's house.
Version 2- added the three screens to the North, fixed the bushes Red Shifter pointed out, and adjusted the camera angle. And removed the trees along the South edge because they block the view.

Next scene- Mt. Tamaranch. I think that's what it was called anyway, been a while since I played this part. Due to the screwy perspective of the series I had to decide whether to replicate the visible tiles, or the shape it was presumably supposed to represent (that is, each 'layer' should stretch all the way around and behind the ones above). The latter would be far too much work, so I just copied what was there even though it makes little sense. To make up for it, I added a crappy background and put some text on the signpost in Paint Shop Pro.

No amount of planning will ever replace dumb luck.
LDD has a hidden Extended mode that lets you recolour bricks, even to shades that don't exist as real bricks. It also unlocks loads of bricks that aren't available in the normal modes, including parts from licensed sets. It's pretty awesome.
EDIT: While I'm bumping this topic, quick preview of my current project- Kanalet Castle. Still a lot of work to do on this one. I took the opposite approach to perspective from what I did on Mt. Tamaranch- all the walls are straight, and I made up ground surface for the tiles that should be hidden behind them.
