For some reason, the game thought one of my companions died, even though that's impossible in normal difficulty and this character was still alive and able to act as a fully-functioning party member. This not only failed their personal quest, but during their part of the ending slideshow, they just got a quick little "_____ did not survive their adventures with you." Also, I think there's gonna be DLC, since there's three planets that are never made accessible listed on your ship's map.
