It's weird that the US has the highest number of good guys with guns in the world, yet the bad guys with guns still keep racking up the body count.
the US has roughly 19million concealed carry holders as of this year, with a good chunk of them im sure not carrying every day at every second (and most of those are spread across several states, not evenly across 50). most people also dont walk around with a gun on them thanks to areas (like bars) largely outlawing them. basically you have to actually have a "good guy with a gun" be present to actually qualify and the numbers right now are too low (getting better tho) to have a large impact when these situations crop up especially when these shootings by and large take place in areas where guns arnt allowed and if youre a "law abiding citizen/gun owner (specially with a registered ccw)" you dont want to break the law.

and if someone does intervene its usually in the opening few seconds of a shooting where the damage is going to be minor and news about it is going to be much more low-key to the point most dont even hear about it. the CDC estimated there are 300-500k defensive gun uses a year. chances are good atleast some of them have stopped a would-be mass shooting already.
