It is more like bunch of psychopaths infatuated with other man sexually because they trigger thei fetish. Like the screenshot is a serial killer that is obsessed with the struggle against death, and the main character nickname is "the immortal" because he survived alot of gruesome wounds and battles. He wants for the MC and him to "struggle" in kill or be killed situation, and when it come clashing, he got sexual pleasure from it.
