Not all paths towards enlightenment begin or end with bloodshed, not even in Yharnam. So too reasoned the Choir, the elite members of the Healing church. They were the ones who discovered the abandoned Great One and in doing so learned that the cosmos was just within reach. By shedding your humanity and all that you once knew, you could ascend to become celestial Kin. The choir graciously 'accepted' many into their circle to transform with the blood of kin, the blood of beasts and the blood of the Great One herself. One such Choir member even found herself forming a strange alliance with the Hunter. He woud bring her subjects both living and dead. She would use those corpses to develop her powers and the live ones to test her theories.
Unfortunately the alliance was shortlived. When the Hunter found out what she did to those people, he broke into her clinic. She called him a fool for ruining her great work. He felt a fool for having spent time trying to save people.. for having mercy.

Iosefka was a kind woman who knew man from beast. She sympathized with those outside during the night of the hunt, but could not let anyone inside her clinic for fear of infection.
You are not her, you are merely an imposter. At a certain point you took over her clinic and made the original Iosefka disappear. It was easy to do, as the real Iosefka never showed her face to outsiders. Now everyone is welcome in your clinic, send them all in. But those who come in never leave. They are no longer patients. They are your test subjects. You turned them into Celestials so you yourself could come to understand the curse of Yharnam and how to transcend your humanity without becoming a mindless beast. Even the original Iosefka became a part of your great experiment. Whether it was all worth it in the end remains to be seen.
