You start at the beginning of the stage if you fail the mission
If only. We established that if you fail you start back before the 10 minute unskippable cutscene preceding the level.
"It is possible to commit no mistakes and still lose. That is not a weakness. That is life."
You have a stress level, which increases when you encounter an enemy, but also if you do nothing for too long.
One time, CE triggered me so hard with their objectively wrong opinions that I accidentally punched myself in the balls.
This level acts at the game's introduction to motion-controls

Edit: Prototypic already mentioned QTEs

When you were partying I studied the bad video games.

When you were having pre-marital sex I mastered the bad video games.

While you wasted your days at the gym in pursuit of vanity I cultivated the bad video games.

And now that the world is on fire and the Bad Rats are at the gate, you have the audacity to come to me for help?
LURKING HARD!
or hardly lurking, see?
Also the person you're escorting dies in the cutscene as soon as the mission ends.
^ Hey now that's completely unfair!

the NPC has a breath meter, randomly panics and can aggro enemies. It also does not path perfectly and will get "lost" if you swim too far ahead, ending the mission.

And in the next cutscene they get brought back alive, show themselves as a bad guy and betray by you by killing another NPC that you really liked.

You can refill the breath meter by swimming over the bubble blower things, but each time you do a dialog box comes up asking to confirm you want to refill your breath.

Any mini-map/quest markers are disabled and you are not told where you need to go. You have to follow the npc, who as previously mentioned moves at a very inconvenient speed.
When does a candle become a blaze? When does a ripple become a tidal wave?
When does the reason become the blame? When does a man become a monster?

Additionally, they kill that NPC by drowning them. Like, grabs them, pulls them under, just sits there staring at you.

Because this is the scene where they reveal they can breathe underwater.

yes, and you can't refill the NPC's breath meter unless the NPC touches it, so you have to make them swim into it. There is also no UI element showing that O2 is low. Instead, the NPC will start flashing red when they are close to death.

Also the NPC had to stop several times to defuse bombs or whatever, and instead of just overriding their normal movement mechanics to go straight there when they get close enough, they usually miss the bomb and have to swim in a big circle and try to line back up, but that doesn't work very well either, so after they spend a good ten seconds trying in vain, they just suddenly snap to where they're supposed to be. And no, the timer doesn't stop for this, nor your breath meter, nor their breath meter, nor can you abort the process to force them to go get air.

You have to escort two NPCs, and if they get too close together or too far apart then you lose.
