The ways ghouls "work" have changed over the course of the Fallout series, and this is probably one the least drastic things they've done. The Fallout show takes place after all the games, so I don't think it's that far fetched to believethat someone either eventually developed a new drug or maybe discovered a side effect of an existing one to help stave off the feral condition
