I thought they were kinda like that in Fallout 1/2
unfortunately i never got far in those games.
I remember starting one of them, dying a dozen times to a scorpion, getting out of the vault, getting to a town with my weapon drawn accidentally and getting shot by everyone and there was no autosaving so i was hosed lol
