I think the joke is either that you're not able to create a character this attractive in vanilla Fallout 4 because Bethesda character models pretty much always look bad.it's referencing the new show. there's a plot point with the main character who is a vault dweller that messing around with your cousin is fine for awhile but that to maintain a viable inter-vault breeding stock longterm, you can't marry or procreate with your cousin.
Or "No Cousin Stuff" being a joke about all the adult mods available for Skyrim/Fallout 4.

And the reply "would it really be fallout without mods tho?" is referring to the fact that literally nobody plays vanilla Skyrim/Fallout on PC. Why would you when there's hundreds to thousands of mods, many of which vastly improve the games' mechanics and looks along with fixing all the bugs Bethesda refuses to fix even after multiple rereleases.

Haven't watched it yet, waiting to binge it with a friend over the weekend. So I guess that one flew straight over my head.
Sure, I've made some decent looking characters in vanilla myself. But it's not exactly an easy thing to do. Take a look at Skyrim, people go gaga over Aella the Huntress and she's as plain as they come.
It's not so much them not being attractive as Bethesda's models overall being blocky.
