I think more inappropriate was that she was his student.In the book she was his student and they weren't romantic. In the movie I don't remember if they ever said she was a student of his.
I thought she was his student in the novel -- engaged to a doctor and not romantically linked to Alan in any way -- and his partner in the movie.Yeah, there's nothing in the movie that even hints at her being his student/junior/subordinate. They're both doctors and seem to carry equal authority in their respective fields.

I've known for years that she's younger than she seems, but my brain still refuses to accept it and classifies her as being older. I think it's honestly due to her attitude more than her appearance. She has this sort of maturity/wisdom to her that feels fairly uncommon, and is pretty cool.
