This is the one I remember being pretty iconic because it was literally the developer of the game essentially getting "hacked" in his own game. Especially since said developer had always put Easter Eggs in his games where you could kill his alter ego. Except in this game, which people still managed to find a way to do.
If by "hacked", you mean "stupidly forgot to set his own invulnerability flag" and left himself open to being killed by an incredibly simple/basic attack, then yep.
If I remember correctly, the guy who killed him just used a common spell scroll of some kind, because he figured it would be funny to just throw a fireball at Lord British in the middle of a speech, because he assumed he'd be invulnerable anyway. Only to be shocked when he actually died.
Arguably it was more memorable for most people because when it happened the admins panicked and basically started slaughtering people indiscriminately because they didn't know how to cope with the situation. So it just turned into a massive shitshow. Probably the only reason it didn't become a major shitstorm is because it was in the beta and they were going to reset everything soon anyway.

I feel like I vaguely remember something about them having an event where massive dragons or monsters were loosed on the world, and it was a huge raid that basically needed pretty much every player to fight, and when they won it led to a lore-based reset of the servers? I don't remember if that was EQ or another MMO. Or just a weird dream I had at some point.

The Sleeper was supposed to be an unbeatable dragon that could only awaken once per server. It would rampage from zone to zone in Velious once awakened and kill anyone it encountered and eventually vanish to somewhere, I don't exactly remember the lore, but its survival or non-death was integral to the lore of EQ.
So, the top guilds of the only server that hadn't awakened the Sleeper agreed to keep it that way, and it stayed that way for a while until one guild went rogue and word got out that they were going to try and kill it.
The other guilds decided to band together and go for it too and they figured out the best strategy was to have hundreds of players at a time each doing single digit damage before being killed and then being resurrected by clerics with their epic quest weapon, which could rez with a simple right click.
And it turns out it was working. They nearly killed the dragon while the server's Game Masters (kinda like GFaqs mods) watched and decided to despawn the Sleeper before he was killed because that would screw up EQ lore.
This, of course, pissed everyone off. Tons of resources were spent and people lost dozens of levels from dying so much. I guess a few days or weeks later, the GMs decided they screwed up and reset the Sleeper and the guilds were able to try again.
This time they succeeded and I don't think it dropped any loot, but I do remember the guy who got the killing blow received the one of a kind title of Dragon Slayer or something like that.
